The players who already have the map start playing, the other ones stay in "-666" mode until download is done. Right?

Basically yes. Players that don't have the map are spectators, however they are able to spawn if they press space (because the server doesn't know whether they already have the map or not. But they will have a black screen until the download is finished.

What happens ? Do the game begin only when everybody's download ends ? or everyone enters the game as soon as he has got the map ?

Anyone can enter at any time and chat, but one should wait for the download to complete so one actually sees something (there is a download progress indicator and a please wait notice on the bottom).

Waiting for the last player to download it would be fair, but a 56k player would lag the others too much - especially if it's a map like fragcity.

One note about my server: please refer to the maps like http://141.2.16.3:7780/nex/maps/. They are more likely to stay there, plus these directories are not littered with other files.
