Maybe its a older update, since I havent played in a LONG time, but when I did play, I remember being able to change anything I wanted to before I started a game. Now I can hardly change shit.
When did this happen, and is there any way I can get it back?

What do you want to do exactly?
Afaik the menu options didn't change that much. You can still customize your player, graphics, sound etc.
Actually, the menu got clearer, more intuitive and way faster in general.

I'm guessing he's talking about the GUI for weapon balancing. The options are still there... you just have to make a config file defining the values you want and load these values when you create your server.

I liked being able to make insane fights or make the weapons designed for one-hit-tweakers...

Dude.

Open the console with shift+esc, type in g_balance and hit tab. Scroll up and down with the mousewheel to see all available variables. For example to tweak the shotgun refire, it's g_balance_shotgun_primary_refire 0.01. MUCH better than the shitty menu we used to have, which by the way added 10 seconds to the Nexuiz startup time. :/
