You didn't turn off chroma subsampling. When I do that:

http://mensa.ath.cx/~polzer/temp/irony4m2opera100.jpg - 1747kB 100% JPEG after turning off chroma subsampling (but difference checking still found minor differences)
BTW, I just committed an engine patch to turn off chroma subsampling. Now JPEG quality loss is MUCH smaller.
