Basicly try to ping the server in the shell and see if you also get such packetloss there. Basicly there is nothing any game can do about that kind of packetloss if its from the connection.
If its a bug it might be fixed though..
Then are you sure you installed the hotfix correctly? Do you see the right date in the console?

I did the following in console:
vid_vsync 1
cl_netfps 25
The lower the cl_netfps, the worse the lag. 25 worked rather nicely. Thanks a lot for your help! I can enjoy Nexuiz again!
