1) When it first happened.
2) Switching weapons with the glitch. I had another showing after the weapon had switched, but it didn't come out (weird...) Anyway, the next weapon had it as well....

No other flashes are in that savegame.
So if one was visible in the player's face, this was clearly an entity networking related problem, and not a game code one.
